and for address proof can i give copy of my driver license?

Who knows. I know for sure that www.visafile.info used their own consent form, NOT an IMM form, and received GCMS files without an issue.

I have no experience filling out this form - GCMS service www.visafile.info emails consent form that is filled already, you have only to print it (without filling).
