What is the best way to go about the medical exam - send away app now or after the exam?

Do the exam just before mailing in your application. The DMP should give you a piece of paper to include with the application verifying that you've done your exam.

Agree with isometry on the medical exam. Regarding your question on double checking what you need to mail in, the link you mentioned has all the forms and there's two checklists (one for the sponsor and another for the Principal Applicant that is region specific toward the bottom of the page)
