As you can see, there isn't a form any longer. Her DMP should be updated to the changes, but I've seen posts from others that the office are unaware of the changes. Just politely tell the clerk or doctor Appendix C is no longer offered. If they insist it is, ask if they can email you the form.

I don't know why the DMPs are so weird about patients bringing the Appendix C with them to the office. Mine told me if I couldn't find it, they would print it off and I could fill it out while I waited. I guess some of the doctors are lazy....... sigh.

Thanks I am confused though, I never saw this link on their website for New Delhi and it says to send the application to the Canadian High Commission in the Delhi office. Should I be sending it to CPC-Mississauga or the one in Delhi?
 
I believe that's more for the doctors than you. The doctors send it to New Delhi. It will be matched with your application when it's forwarded from CPC-M.

Also how do they match the application, I do not have a file number or a UCI number, is there a way for me to get it now?

no - you don't need that. When you go to the doctor, he will give you a sheet of paper with a number, your picture and his signature. Add that to your file, and he has the double that is attached to your file when he sends the medical results. Like this, it's matched!

Good for you, but why are you even posting in this thread? You're just fishing for +1's? Really?? This is Jan applicants.. not Jan PPR Approval thread. As far as what you say and asking for a +1 on something unrelated, that actually gets a negative vote from me to be honest. (Not negative because faith, I do believe what you say) Please use the thread for it's intended purpose.

What a strange thing to post. Whilst I agree with what you say why are you fishing for ratings on a forum?
 
Okay guys I am oficially a January 2013 applicant!

I sent my application via next day courier today and the post office says it will be received tomorrow morning (January 30, 2013). My husband is American and we applied via an outland application. I hope all goes well!! ;D

My husband's medicals were done on December 21, 2012.

Nic, Amikety! I'm bound for Brampton, Ontario. It mught change in the future. If so, I'll let you know. Also, Manila has well, one L. I noticed that mine had two. :) thanks Amikety!
