Is it advisable to apply for TRV and Open work permit together?

My wife is in Canada on a Visitor record expiring December 05, 2021. Her TRV- multiple entry visa sticked inside passport is expired. I'm also planning to change condition on her status from Visitor Record to Open work permit.

1) Is it advisable to apply for TRV and Open work permit together?
2) I am unable to find the questionnaire for TRV?
