Both the document checklists for the sponsor application and the common-law application mention the IMM0008 need to be included. Do I need to include this twice separately in both applications or do I fill this out once and only include it with the sponsor application?

There is no separate "sponsor application" or "sponsor checklist". PLEASE make sure you are using the correct forms to apply.
