How to submit use of Representative form

Hi experts/seniors
Who should submit "use of Representative form" to ircc via webform; the sponsor or principal applicant?
Because when we submit any document or query it asks applicant name, email, UCI etc ..it also ask who is doing this query ...please guide..highly appreciate. Thanks

I submitted mine using all the PAs information when filling out the webform even though I am the sponsor. No issues.
