How to order CAIPS Notes ?

Needed to confirm about how to fill the main Request Form for requesting the CAIPS via a relative in Canada.

I am from New Delhi (NDVO).

form 1- http://www.cic.gc.ca/english/pdf/kits/forms/IMM5563E.pdf
the main Request form.

form 2 - http://www.cic.gc.ca/english/pdf/kits/forms/IMM5475E.pdf
for giving the consent.

Please open these links/forms before reading the questions.

or any other form/document as well?

Q 2 --- in form 1, Section A - Point 1, 'Access to Information Act' or 'Privacy Act'?

Q 3 --- Section A - Point 3, response by Email or Paper format ?

Q 4 --- Section B, Do we have to Choose "Electronic files" or "Physical Files"?

Q 5 --- If 'PHYSICAL FILES' then ''IMMIGRATION FILE OVERSEAS" or which option

Q 6 --- If "ELECTRONIC FILES" then (A) Status of an application or (B) Immigration file in Canada

Q 7 --- What to mention in the "Location" column?
and I have got an Immi. File no. only.

I am applying CAIPS through a relative in canada.

I have a few questions:

a) on top of the form
Choose one
1. I authorize citizenship and immi canada and canada border services agency....
2. I withdraw my authorization to release info...
(which one to choose?)

b) on the bottom of the form
7 no. point Your declaration
If you are withdrawing your application...
(should I and my spouse sign there?)

-i am in process dating 24.5.2012, so, its been a month now, should i order the CAIPS now or will be better to order after 1 more months as CAIPS may extract more info by that time.

- whenever the time maybe, should i request my consultant(from CA) to do it for me OR i should do it myself? whats more convenient?

Dear Qorax & my dear Seniors,
some questions are needed to be solved plz;
1)without a copy of PR Card & Covering Letter can i get CAIPS Notes?i mean the whole package is IMM5475,IMM5563B,payment slip and a copy of utility bill is acceptable?
2)can i send an e-mail from my id or just my uncle will send the whole package to apply for CAIPS?
3)If i have send the whole package through my id then?
4)i have sent the above said documents to first london-im-inquiry and after 2 weeks i found that those should send to ATIP-AIPRP then i forwarded the same requisition to ATIP-AIPRP,,was this act ok or not?
Seniors,i am new to this forum and need the specific answers as i stated above.i hope u will help me out from this confusions.
