once they approve your visa, you have until the medical expire to land in canada, if you don't the opportunity to land in canada expires.
What expires is called a PR visa (i think). The visa allows you to land in canada and get your PR.

Once you land in canada, you become a PR. You can renew your PR status but living in canada for 2 years out of every 5 years. This means, if you land in canada and come out immediately and want to go back after 3 years and 1 days, you can't because you won't be able to make the 2 years within the 5 years.

So ok once I land for even 1 day there before my medical expires and then comeback to USA, I still have to go back to Canada before the 2 years for another day right?

From the day you land, you would have to be in canada for 2 years out of the next five years. You have to be in canada for a total of 2years and this doesn't have to be continuous. you could go to canada one day and go to US the next day and then repeat this for 4 years (but i am not sure how partial days are counted).

You can go as many time wherever you want to after landing, just ensure to keep the PR status, you stays in Canada for a total of TWO years out of FIVE. You can do the math on that, the days are counted on the days you stayed there physically not came on one day and left on the same day can be counted as one day, coming into Canada on one day and leave on next day can be counted as one day of stay.
