help with spousal open work permit application

Hi, guys, I am working on the inland spousal sponsorship application for my wife. And off course we also apply for the SOWP (there's the main reason why we choose inland).

But I am so confused how it should be done.
1, Is the application form IMM5710e ?

2,IF it is, how to choose in question 3:

(I am applying for one or more of the following:
A work permit with the same employer
An initial work permit or a work permit with new employer
restoration of your status as a worker
Temporary resident permit)

I thought only "Temporary resident permit"fit? because my wife never worked in Canada, but when chose it, it is requested that I have to choose the first option"A work permit with the same employer" too.

And is this form ( which we will send with the sponsorship application to CIC) the only I need to apply for the work permit for my wife?

Hi,
I will come to Canada on a Visitor Visa soon. I will apply for Inland spousal sponsorship along with OWP. I have a question regarding the same.

Do we need to mention 'To' date for current activity/occupation? Can I be on leave from work for a foreign employer when I file the application? Any help would be really appreciated..

Also do we have any link or document which says person who is applying for OWP should not hold any current employment?

Generic form, generic option. None of the other options apply.
