" The consent provided appears to have been tampered with. To be valid, a consent must be signed, recently dated, contain the printed name of the person giving consent and identifies you as the designated individual. In the event that the consent provided contains electronic signatures, please provide the audit log from the consent form in order to confirm the signatures provided."
So it seems an audit log is needed. Maybe someone else can confirm this that have done online signatures.

B : empty
C3 : Sponsor
C4 : Sponsor signs
If you have the latest version of PDF you can sign the form online along with the sponsor no need to print or scan....

Thanks, you are right.. I'll do it today 8)just a square said:I would. It's only 5 bucks - wouldn't hurt much. However, it takes 30 days to receive the notes and you might get DM by the time notes arrive.![]()

They asked my husband to re do Medical 2 weeks ago, and he had them done. Now Medical is passed, and Background check changed to: in progress, a day before they asked him to get Medical, and that is about it...
Now nothing is happening...

just a square said:But the notes would come before Mid-Dec, and then, if they show nothing, it's easy to just order new ones.

(Note that most of the people from my group are either at the final stage of their app, ie parent getting PR). Anybody has any comments on what they can read from this?
