FSW 2014 :: Generic Application form query

Hi All,

Need your help. I am applying for Canada federal skill worker program.
I have below query's regarding "Generic Application Form for Canada":

1.) Immigration Office requested for processing this application. :
I am a Indian citizen, currently working in US on H1B visa for last two years. I want "New York" as my Immigration Office, but I have heard that this office currently do not process federal skill worker PR application. Is it true. Has any body specified "New York" as their Immigration Office for their FSW application, recently?


2.) Passport (Country of Issue )

I have a India passport, but it was issue in United States. So, in this section should I mention India or United States?

The Country of Issue of Passport will be INDIA and NOT USA. Please rectify it asap.
