NOC 4011 , 19th August Application Date, CC charge 19th Nov. PER wait is killing me. I tried to reach to CIC call center as some members did to grab UCI # but no luck. I think so far they must have made decision but this anxiety is killing. ???

CC/DD charged 18th Aug week guys chill today it's the day for PER for us yes including me it's today or very soon
Since people with cc get to know about payment earlier than dd it makes them more anxious though I'm also same week applicant paid by dd so dear member what I meant is that as per the trend we all get the PER nearly same time but you guys get to know payment v early so it doesn't make any difference we all will get it ASAP good luck to you and me and others of same dates stay optimistic and have faith ;D

1) On what name should DD be taken for RPRF Make it payable to "Receiver General to Canada" or "Canadian High Commission".
2) Covering Letter: Can someone share a sample cover letter and Affidavit (both) for the below scenario please

3) Apart from Covering letter, is there any Affidavit needed? Also what should i include? Will the below suffice or should only particular documents to be sent?
- Old Passport bio pages
- New Passport All pages with notarization
Photocopies of both Passport will do, Notarization or affidavit is not needed if they are in English.
4) What address should i send the documents to?

Dear friends,

I have received my request for PCC, medical and RPRF. However, I recently realized that I have mistakenly mentioned "NO" to 6D "Schedule A", whereas I was refused a US visa 2 years ago. I totally mentioned "NO" inadvertently/by mistake as I missed that 6D mentions other countries as well.

Is there a way out to the situation now? I feel like writing an email to LVO, apologize on error and request them to allow me to send new Schedule A along with PCCs and RPRF. Will that be successful?

Does this mean that my visa application will be rejected even after letting them know now?

Anxiously waiting for your kind responses.

In the, Supplementary Information – Your travels (IMM 5562) form ... There is no place to sign the document ... Should I sign at the bottom along with the date or leave it as it is ? As it is mentioned in the checklist that you must sign the forms along with the date ... Seniors please advice ...

You can do it during mr stage.

But wont it create a problem.lets suppose they do a random check on my file and come to know from my employer that i am not working anymore..So they should have the info of the latest employer...

No need to send any apology email to LVO.

Wait till you receive PER, after that send amended form along with Cover Letter mentioning reason of why you are sending amended form.

If there is no place to sign that means they don't require your signature and your are not supposed to sign it.

Further queriey, When you say Photocopies of 2 passports, means i should also send again all pages of first passport?
