Personally,i feel since you did not get her birth certificate on time,you should have written/attached a cover letter to explain your situation.This was stated on the document checklist.
 
kewl, the info you are giving them, that IELTS RESULTS MUST BE IN A SEALED ENVELOPE, IS WRONG.

It doesn't have to be in a sealed envelope, it just has to be the ORIGINAL results. copies can be submitted upon certain conditions.

Really dude, don't scare people with unverified information, a lot of people here are on edge already, no need to be spooking them further. ;D

Actually , I ordered for second original copy while I registered for the IELTS test.

So answer is yes we have to send original copy to CIC (in sealed envelope, otherwise the result report cant be accepted as official document)
another original you can keep for your records and obviously get photo copies of it, but it cant be considered an official report.

My IELTS report was green too, but they do able to issue 2 reports or more.
As my first report was sent by snail mail and eventually was lost, I requested another one, paid extra for carrier service and received it.

I don't think they have different forms of report in different countries. But to make it sure better to clarify it by calling to the respective office.
Anyway it is too late to change anything.
 
Just in case persons are worried ...my IELTS were not placed in an envelope neither was my ECA. Have a good weekend all, hopefully good news awaits next week.

If you can get that daughter of a CIC officer to sign a legally-noted affidavit that she saw the original IELTS included in your documents, then you have a fair chance of getting your application reconsidered, prepare this one and have a copy ready for resubmission.

I am a foreign national as well, I've been living in the USA legally since 2007, my question for you is, are you a permanent resident of the USA?, are you a green card holder?

This will make a huge difference in your case. Please let me know so I can be of further assistance for you, I did apply for the FSW back in 2009 through the Buffalo Office in NY, and after almost 18 agonizing months I got my file rejected, and returned to me. I would like to prevent you of making the mistakes I made a couple of years ago.

you need to send your complete file to Centralized Intake Office (CIO - Sydeny, Nova Scotia) while mentioning your visa office CPP-O in the application form.
