It should be on Rs 100 India Non Judicial Stamp paper.
It should mention the following
1) The name of your wife
2) Name of her Father and Mother
3) Her Date of Birth
4) Her Place of Birth

wait a minute! I also submitted extra document in support of my application after I realized I didn't include the Schedule3 Form(Your Travels). Additional document was received May 21st. Just saying though..

How can we be sure that Sachin's original application has gone thru the Cap phase? Sorry I am not trying to be negative but I would want to be sure and not to assume based on any hypothesis.

Its CIC not your lawyers office where they will put in place all the missing documents or the Jigsaw puzzle. when they open the envelope they expect a "complete package" . whatever is incomplete they will simply return it just like the case of sachin1983. Until and unless you dont get UCI you can never add or expect them to add the missing documents to your file. its very simple.
For example , if today i send 1 application form , then tmrw i write a cover letter askign them to add my ielts results and then 1 week later ask them to add my Evaluation, .
In all the above case everything will be returned one by one ( depends on when they open each package individually). even if u write a covering letter written by Jason kenny himself.

Believe me everything about CIC is a touch-and-go. I really hope outcome will be positive.

True story. Unless you have an UCI they will just send back whatever you send.

No one is sure...we are all assuming!

Simply because his original application did not get returned back to him.

In my humble opinion....I think what happened here is that if he does not have an UCI yet, the CIO just sent the extra form back without even checking his file. I don't see how this is evidence of any application passing the cap filter. The question is, does he have an UCI? If he has an UCI code, then it would make sense to think his application is ok and did not need the extra form. Hopefully he has one and included it in the cover letter.

Great, thanks! So, my wife's father need to sign it? I read somewhere that the letter should be from notary? Another query: my birth certificate is pretty old and doesn't mention my name (as I had no name when I was born ), so is it advisable to submit it or get a similar affidavit for myself too?
