Important:

My application was receivd by CIC on May 16h, 2013.

Validity of 2 years of my IELTS is going to be expired on June 12th, 2013.

As per CIC guide, its acceptable.

Any conment?

Oh you are in a tight spot. well if your application has been reviewed by them by this time then you are good to go.
if they review after June 12th, then they have reason to return it back or ask for new IELTS but you also have chance to fight back based on the fact that at the time of submission, your IELTS was valid.

you said its acceptable as per CIC guide but beware they always play with words. they always use lawyer's language to communicate .

Yes. It has to be valid when it is received at CIC. So you are safe.

Yes, they do accept the old generic form :)

Thanks Marta for clarifying the situation.......

You are quite fine...as long as it gets to CIC at least 2-3 days BEFORE it expires then all is fine.

User Anac has contacted me...He is still in the game! He sent the new schedule 3 form, so he is only a little concerned about the old generic form. He hasn't gotten any feedback as yet. His application arrived at CIC in the afternoon of May 9th.

Happy Waiting...........

hi everyone. I've got some tricky questions. I'm a phd student in canada and i'm staying here with student visa and I've also job experiences in 2147 and 2174 . my supervisor is the manager of a company and has employed some of his students before. could you tell me if it's possible to ask him to give a job offer? is the job offer something special and it needs to be certified somewhere? or he can just simply write a job offer letter for me? does is need to be full time or a part time job is ok?
FYI, I have a good funding for my phd

WOW, arash, so you basically fit all three categories!!! If I were you I would forget about the FSW 2013 2147 & 2174 categories as I am sure these caps have been filled already. You are then left with applying under the PHD stream....this is if you have completed at least 2 years already or you can go for the arranged employment option. Yes your boss would need to do quite a few things in oder to give you a job offer. There are certain criteria's that must be met, but this may be the fastest option available to you right now.

See link below on getting a LMO (arranged Employment and Phd requirements.

2147 and 2174 are hot NOC. You are advised not to enter the game now for these NOC otherwise it is quite likely you may lose.

Added. Welcome :) :) At what time was your application received?
