Form IMM 5409 common law status declaration

I posted this earlier in another thread but I think it was the wrong place so I'll try again. I'm Canadian, living in London, and I want to sponsor my British partner of 10 years. We aren't married so applying as common-law. Am I correct in assuming that I need to have form IMM5409 filled as evidence of our relationship? If so, can I get our family lawyer to do this for us or is there someone else that does this in the UK? Thanks for the help.

The application guide stated that it should be included if your spouse or common law partner is your co-signer. However, I just included it anyway as additional evidence as I read here that sometimes it is requested later on. So my advice would be to just include it, it can't hurt. As I am not from the UK I can't help you with where to get it notarized.
