Hi everyone. Just wanted to ask how you filled out form 5710. This is for my spouse who got OWP but due to passport expiration date was only issued until the validity of his passport. We got the new passport and would like now to apply for OWP extension. My question is about Form 5710. In No3. which would be the best answer? I am applying for:
- a work permit with the same employer
- restoration of status as a worker
- initial work permit or a work permit with new employer
- temporary resident permit
I know restoration is for those who lost status so definitely not that one. Do i also include temporary resident permit? Both his work permit and Passport counterfoil just says until 2018...Please help. Thanks!
