First and Last name gap

My Passport has First Name and Surname ( i.e Manish Singh) but my WES metions Surname in bracket ( Singh) because all my educational certificates except passport has only single name i.e (Manish)

Should i declare in the PR application that i was known by different name, if yes entry to former name field in the form is not accepting only single name

Please help. will this affect my PR filling. I have got ITA invite.

Yes you must declare other names and also include affidavit for one and same person.

If you passport has only 'given name' then enter give name in surname field and leave given name field blank. Check http://www.cic.gc.ca/ENGLISH/helpcentre/answer.asp?qnum=014&top=4
