has anybody asked them to send a duplicate copy of the report? just sending them an email with the name date of birth ect enough for them to send a duplicate?

Yes, I am in the same situation.

I had sent the card which bank cancelled in the meantime and sent me a new card.
I was late by days to send them new info and I am back to square 1.
I am not even sure if fingerprints got accepted.
Did you call them and checked on fingerprints?
 
Opening packages of April 17 and processing of April 9th

I did not email them anything. I just faxed them the new CC form and attached an explanation letter to the form (faxed both of them to the number they gave me: (304) 625-9792)

I replied to your PM. Essentially do NOT do both (fax and mail). My "decline letter" specifically said this. My suggestion would be to use the above fax number as it is much quicker (i.e. just fax the new form and letter - otherwise it will confuse them).

received my 2nd FBI. i applied 2 times lol but 2nd one damaged with water from bottom. i sent my prints on 24 march and they mail my FBI on 18th june and received in Calgary today 25 june 2015
 
I was informed by FBI this morning over the phone that my PCC has been mailed out today, June 25th (recd by them on April 7th). Does anybody have an idea of the transit time of their mail to Canada? Alberta?

I called customer service rep today and she told me that they put me in the April 14th' que which is fair considering I will not be made to wait another 12 weeks. She asked me to check again in the middle of next week.
My documents were received on April 6th and I lost a week considering I had to resend my credit card payment form.

Application for PCC was received by FBI on April 03, 2015
CC payment was Processed on June 16, 2015
FBI Mailed PCC on June 22, 2015
Arrived in the mail today June 27, 2015

I'll have to mail it to CPC-Ottawa on Monday when the post office opens.
 
If I apply for FBI PCC while in USA do they process it faster. Does anyone has any experience with FBI PCC while in USA?

The processing time would be exactly the same no matter where you send them the app from. However TRANSIT times will vary significantly depending on where you apply from. Obviously transit times for them to send it by mail to you, once they have processed your application, would be faster if you were in the States (roughly 4 to 5-ish days). I'm on the west coast of the US and I got it in 4 days (from the date they mailed it out). I've seen on this forum that it has taken 12 to 15 days for people to receive it in some international locations, as the FBI only sends it by regular mail.
