For the sponsor/sponsored contact information they require a section on fax number -- but I don't have one nor have I ever used one?

No, its not important... I don't have Fax also,
and I did not include any....

You should be OK if you don't provide Fax number

Yes, try to read all the questions and don't leave any blank space
always answer N/A where its not applicable.

In some case the form will tell you what to answer in case of not applying to your case,
so pay attention to each single question in all the forms.
Not read the questions well can lead to forms not fill properly and therefore the risk
of the forms returned.
