Checklist items that don't apply to you - check and "N/A" or just "N/A"

For items on the checklist that don't apply to you - for example, where it says "additional information about your children" or "medical examination" - Am I just supposed to print "N/A" next to those items, or do I have to check them AND print "N/A" beside them?

Have you considered drawing a diagonal line through the checkbox, killing two birds with one stone as it were?
