Canadian passport identification documents

Hi All,

Im applying for canadian passport,in the application we need to provide the identification documents like health card, ontario card or drivers license.

all I have is a health card, but that is up for renewal.I renewedmy health card but the physical card wont be renewed untill 4 to 6 weeks
However, I do have a temporary copy of card and old health card as a photo proof.

so would I able to use my health card with temporary page health card tigether to present identity proof in passport.
Any ideas?
