I wonder if we get to pick who is coming to Canada with us, and who isn't, when approved for Start-Up Visa program/Business Investor immigrant programs?

If you wish, you can include your spouse (if you have one) and your dependent children (if you have any). Even if you decide not to bring them with you to Canada, they must still pass the medical in order for your visa to be approved.

No - you cannot include your parents or siblings in your application. They are not classified as dependents.

It may be possible to sponsor your parents later once you've lived and worked in Canada for several years. If any of your siblings are minors and dependent on your parents, it may be possible to include them in the application to sponsor your parents.
