Compose an explanation letter stating the above. Just mention that your employer is not willing to issue a letter and you can't seem to find your offer letter either (I would cut the latter out, don't direct the blame fingers on yourself, although you should have been careful with that offer letter).

Don't worry there should be no problem. The officers will understand, there have been cases like yours. If they need to investigate more they will call the office of your work or they will contact you for more clarification.

Rule of thumb; make sure you make a copy of all your documents for later use. It only makes sense to have a copy (physical or digital) for your own records.

While a letter of explanation is necessary, it is insufficient. You will have to provide alternative documentation of dates of employment, job duties/responsibilities, and salary/wages.

Canadian experience - you can provide your T4s, NOA, record of employment, first and last paystub... as for job duties/responsibilities, try to get a former colleague or co-worker to write a letter. It does not have to be on letterhead, but should include his/her contact details and a business card.
