There definitly is a new form. I just click on that link and it gave me the new form.

Only when I went into the form and then refreshed the page, the new version came up. Odd... Glad you shared it here on the forum, otherwise I hadn't known about it!

Its very important to use the current form - Ive heard of so many stories of applications being returned because it was not on the most up to date form.

I just got an e-mail from CIC that my application has a message. and when I checked it said I am issued a new BOWP and it will take up to 6 weeks for me to receive it. I used the online application process. Documents I submitted were: app form, picture, scan of passport.
As I have been seeing a lot of people are confused about letter of explanation and job offer and other documents, I did not submit any letter of explanation nor my job offer.
My time line is:
Applied: Apr 2
Notification of approval: May 13
Actual work permit in mail: Waiting.

Congrats orko! Yeh, curious to hear if you had recieved your AOR before you applied?

Can one apply for BOWP if they have received the PNP approval letter? or is this restricted to only when you reach the CIO/CIC (federal) stage post AOR?
Any help will be appreciated!

If this decision has not been entered yet, for the purposes of this Operational Bulletin (OB), a “positive eligibility decision” will also be considered made when it is confirmed in GCMS that:

PNP: the “Eligibility EC-QC/PNP” status has been set to “Passed” or the Acknowledgment of Receipt letter/e-mail has been sent by the Centralized Intake Office (CIO).

It clearly state that an AOR needs to be sent by CIO. You can try with the PNP letter and write an explanation about AOR. If your circumstance are reasonable, CIC may still consider it. However, the rule says AOR from CIO.

Orko: Can you please share details on how you had successfully applied for your BOWP? did you get your AOR first before you submit your BOWP application? How about the expiration of your previous work permit? are you on the same employer or new employer?

I am currently on an LMO Exempted work category with work permit that has been renewed and will expire on March 2014, and I really wish I can change employer... I'm not sure how to apply to get the BOWP.... I submitted my CEC on April 2013, waiting for AOR.. Needs some advice from you seniors.

I just want to confirm that the form I need to fill out is the IM5710 and it is 5 pages long! Is this the only form I need to fill out and post for my BOWP??

Yes, I received AOR before I applied for BOWP. For CEC, receiving AOR is the requirement to apply for BOWP (there is diff requirement for other classes). However, in case some one didn't receive AOR, they can apply for work permit with LMO (which is not BOWP).
Also, I did not have to submit my copy of AOR.

For those who are applying online, the key is to make sure you have the right answers for the online wizard. I took help from this forum (by reading previous posts). That cleared up some of my confusions. Fill up the form correctly. Make sure you have received AOR before (not assuming that you will receive it after). These should be enough for a successful application! Good luck guys!
