My visa application was submitted in August 2023. Biometrics and medical were completed, and eligibility was marked as completed in April 2024. The background check was updated to ‘in progress’ on April 19, 2024. I also received a pre-arrival letter on April 18, 2024. Since then, the application has been stuck at the background check stage. How long does it usually take from this point?

Hard to say without knowing what you have applied for.
Note that background check can take anywhere from a few short weeks to many months to complete. The pre arrival letter is largely an administrator step and doesn't mean your application has been finalized.
Pakistan definitely has longer processing times. I would suggest finding a thread with those applying for sponsorship from Pakistan to get a better idea about processing times.

Most likely you are in a comprehensive check which could be quite a long process.
What is the point of GCMS notes if they are aware that they are in background checks. Better option would be to compare to others sponsorship a spouse from Pakistan to see if their processing time is abnormal for the country.
Usually being asked to redo the medical does mean that they are mostly ready/preparing to issue the final decision - and probably that other parts are okay. It's not a guarantee though.
