Annexe A needs to be cut out into a seperate document and filled?
Or just take a print out of the whole email and fill it?

Oct 16th 2014 applicant, just got my RFV email today morning (Dec 12th)

Has anyone else applied from the US and can tell me what courier service they used? In the email received it says I have to make sure to use a courier service that can deliver to and from US & Canada.

Secondly, When completing and signing Annex A, I am assuming we have to print and attach entire email to package as well?

My address is still there. I am not a permanent resident or citizen of USA so not visa exempt. Still have to send my passport

HI.. I got the shedule A request... here's wat i did.. I downloaded the form first then den i completed the form took the print out did my signature and scan it again... then i saved those form and sent it to them... hope it works...
