Schedule A imm 5669
Additional family information imm 5406
These two forms are required from your spouse and can be scanned
Or photocopies copiez.

Now my question is they require it from my spouse, then
Whose name will be put in the family information form
Applicant column and who will sign at the end of this form.

And for the form imm5669 who will sign at the end ofform me or my spouse?

IMM5669: if she is not principle applicant she will be marking spouse,common law BOX, not principle applicant
