Medical exams for a study permit need to be done in advance or there will be a request sent after submitting the study permit application?
Study program 4,5 months in lenght.

You can do either. A medical exam before application is called an upfront exam. If you are applying under SDS, an upfront exam is required. For regular application, you can do either upfront or just wait for instruction.

Even with a regular application, you can do upfront if you want. At least that’s one part of the application to get out of the way, especially in these time of different delays.

the study is less than 6 months so it may not even need it.
Also if a person wants to do upfront is there medical forms or medical papers or reference?

No, just go to the designated Medical clinic in your home country and tell them you would like to have an upfront medical exam.

Yes. They may ask for your passport, though, or any government issued ID. When I did mine, they only asked for my passport and for which country (the medical clinic in my home country is catering for both Canada and UK bound applicants).
