Email and you can also see in GCKEY online, under the application.
They don't send anything to the physician. When you find a physician you have to take the email attachments to the office (and anything else they have requested). My doctor used an electronic system so he didnt need spouse's passport sized photos or the paper that came attached to the email. He then sent everything electronically, and he told me they would receive in 2 days.

I have received the confirmation that the file has been transferred to UK immigration office but haven't received the confirmation that it is being processed. I emailed them and was told that my file has been assigned to an immigration officer and is waiting an initial review. Does this mean that the file is being processed or still in queue to be processed?

From comments on here some say they did get it and others say no. If you don't get one it doesn't mean there's a problem.
I believe it means it is in queue to be processed. I had emailed them about a correction on my form and they replied 2 months later saying file has no updates. I did not receive notification that it was received. Only an initial email that it was forwarded to London VO.
