If you were truthful in your app, your father supposed to get the medical. Was that happened?
Mine too, so is this SCI FARLEY guy legit? Did your passport make it to IRCC and back to you? I’m worrying sick about the whereabouts of my passport.
Regardless of the intention of your father, he would have had to do all the stuff your mom did such as providing police certificate, marriage certificate/decleration, medical exam etc. So, it's more or less a matter of him changing his mind and wanting to accompany your mom. My personal opinion is that it should not be very difficult to change that "check box". I'd ask that question directly to your local visa office and get that answer asap before they issued passport request. If your mom already sent her passport and waiting for COPR, then it could complicate things. But, it never hurts to ask the question. If you don't ask for it, you are not going to get it for sure. Use IRCC webform or direct email to your local visa office asap.

2) Yes. You can prefill the return address. If you don't do that, CIC staff will fill it out.

just when i thought we were about to receive DM on PA, we are asked to provide additional information - proof of relationship because apparently some info is missing on my birth certificate, it's hard for us to get any other sort of proof at this point, I'm not sure if anyone had similar experience, what do you suggest me do at this point other than explanation letter (which we will include regardless), and will this just be the end of my application

And also called the supervisor in the Edmonton office (the number was provided by @bdidol ), she said its not her office where this request would go but she took my phone number and said she will try to find out which department/office it would have gone and she will give me contact information from that departments (hopefully she could find that department).
I’ll share the details as soon as I get any email or call back from that supervisor.
DM on primary applicant (PA) or sponsor? If it's on PA, you should see passport request within the next few days. As for biometric, they could decide to use BIO your mom did for super visa. That could be the reason why decision was made without requesting BIO again.
Yes send 2 in total, one carrying your passport and the return envelope and don't write the address yourself fill out the form at the bottom of the letter that was sent to you
I did t get any form attached . My letter actually says write return address .
Weird then follow the instructions they gave you, mine had a form at the bottom.
