when in doubt just give whatever information you have of ANY medical problem that you have. And then let IRCC decide.

Having said that some of us have experience with treating patients and even doing these medical examinations for immigration and employment purposes and know what conditions are typically what the organizations are looking out for.

i.e. My dad’s case, he had skin cancer almost 6 years ago and was operated and is doing ok.
We mentioned this to the PP on his super visa medical and reporta were provided.

We will mention this to PP on his PGP medical too. But i did not mention it on the form 5669, which I thought is only asking for present serious conditions, may be i misunderstood but for sure there was no intention of hiding it as they already have these details in his super visa’s medicals.

So this user is asking the same thing about his/her mother, if they need to mention it on the form 5669 or it should just be mentioned to the PP once they do the medical exam?

It should be mentioned in the form AND during the exam. If it's not mentioned in the form, it's withholding information which as per the site below could be a reason to be qualified as inadmissible to Canada (see misrepresentation) :

Did your father have Basal Cell Carcinoma? BCC is generally not considered a "serious" cancer.

No, it was melanoma. And i know it is one of the dangerous types of skin cancers.
But i did not mention it on the 5669.
But for sure it is not to hide it, because I have asked my dad to get all the reports from his dermatologist who was following up with him since 6 years, so that we can show this to panel physician.
And it was mentioned to the panel physician and reports were provided to IRCC at that time when he did his medicals for super visa in 2017.
