Application accepted but no visa

I got an email with subject application accepted, and was asked to pick up my processed passport from the visa application center in Lagos. Today I picked up my passport with no visa or letter, just a note that says to send an email to Nairobi visa application or whatever. Can anyone tell me what may have happened?

Can you please copy the text of letter to better explain. Perhaps they are asking you to enquire with Visa application center to check on what went wrong
