Medicals passed, but they have asked to upload IMM5475 form.

I need to enter the visa office details in this form. I emailed to CPC-CTD-Ottawa@cic.gc.ca yesterday asking about the same. Is there any other way of knowing these details?

Edit: I have just submitted a web form. I hope I receive a quick response as I have to submit this form within the next 7 days.

hello everyone, does anyone know what it means after submitting ADR and status of ADR changes to "not applicable" most people changes back to "we do not need additional documents"

Because CIC wants more information? In my case they asked for Schedule A, which was already included in the LEO, but perhaps they missed it.

You mean yes to a statutory question - which we did because we had a visa from Canada cancelled, because we accidently applied twice, and thus had the first one cancelled. Was not sure even if we ought to answer yes, but thought it was safest to do it just in case.

Yes I had meant to say 'when you do not answer no to a statutory question ' changed the whole meaning.

they requested rcmp check which was never requested in initial application
