Hello,
So My wife and I got married 3 months ago in Dubai. I have Canadian Citizenship while she has Pakistani Citizenship. If we got married in Pakistan, does she need to change her name in her NIC card?

Its not mandatory that your wife change her surname in her previous documents like travel passport, NIC card, etc.

Your wife could apply for spousal sponsorship using either her maiden name or husband surname.......however, if she changed her surname after applying, you must inform IRCC.

Its required in the spousal sponsorship forms IMM5669, IMM0008 and IMM1344.... to indicate your full family name (surname or last name) as it appears on your passport, travel or identity document.

Thank you Wakki for responding. I told my wife that she doesn't have to change her name on the NIC.
