I used the old form as I had my husband sign a few last year that I could send at different times. I hope they don't come back and say that I needed to use the new form. But I did see someone else that requested their notes using the old form and they got a reply a few days later asking that the person send the newer form, so I hope that if they were going to ask me the same they would have requested it from me by now.
I requested mine on the 14th feb and i received an email on 16th march to resend the forms again using the newer forms. What a bunch of time wasting ppl. They took 30days to reply me back with this crap.. lool

OMG i just saw the date on the consent form i sent them, it says oct 2016. Damn, its definitely coming back then. Where can i get the newest version please and what date would it have on it?

Isn't it too early to request gcms notes?

You can order them at any point. And there can be some valuable information from the Mississauga stage - they may indicate to AVO to pay close attention to red flags and may flag a file for an interview.
Oh had no idea. Did you see anything interesting in yours from the mississauga stage?

There was no indication that the Mississauga officer was concerned with our case. I mean AVO could see something differently, but at least Mississauga didn't so that put my mind at ease a little.

When applying for notes, it has to be the person that is in Canada that submits the form:
*To be eligible to make a request, you must be a Canadian citizen, a permanent resident of Canada or an individual or corporation currently present in Canada. Please select the category that best describes you.

I think it is when it is processed. It is only $5, so order them every few months. I wish I had ordered mine more often.
