DMV in US have prescribed format which conveys date of issue ,how long and your driving history -demerits points etc that should suffice..

This is the form I used to request Certified Driving History of my Arizona license. Different US state DMV's have different forms. Note that it should be "certified" driving history.

Then I booked appointment in SAAQ Henri Bourrasa in Montreal and showed them this certified letter from Arizona DMV with the original license and they issued me Quebec license on the spot without any tests.

They kept my Arizona license and didn't give it back though, they said thats the procedure and if you want to keep US license, then you have to go the normal way of driving tests and all.
They also added my US driving years to Quebec license years.

Are you moving from USA? From your profile it appears that your VO was ADVO. Anyways, please note that I was able to exchange my CT(USA) license with QC one without any "certified" driving history.

I am not moving from USA. My current country of residence does not have an agreement with Quebec, hence its gets trickier to get a license.
