are you appointing or cancelling representative? if Appointing check on (tick on) "I am appointing a representative" then fill Section AB&D.

Section A:
1. your spouse first last name and first name
2. spouse DOB
3. Name of the office and type of application: PR
4. Spouse UCI number
Section B:
5. representative Last name and first name (your name if you want to be her representative)
6.i. if you are the one Check on Family or friend
7. mailing address, phone and email.
8. Representative Sing(your sign)
Section C: this section is to cancel representative if you already have one. otherwise ignore
Section D: your spouse Signature
