Hello, we're working on PR under Common-law Partner in Canada Class and have a few questions for these forms. I really would appreciate all the help I can get!

Question 1) At the very end there is a consent section that we're confused about. We're asked to check yes or no boxes at the end of the following, but not sure what exactly it means. It's pretty confusing and is it something very important as in should we just tick yes?
"Consent to release information to intended province/territory of destination
I, (first name, last name) ____________________ on behalf of myself and all dependants included in this application,
authorize Citizenship and Immigration Canada to share the information collected in this application as well as supporting documentation with Provincial and Territorial authorities with responsibility for Immigration for the purposes of the Immigration and Refugee Protection Act.
Consent to release information for Evaluation purposes
I agree that the information contained in this application related to my intended occupation, education and work experience may be shared with prospective employers in order to assist them in hiring workers"

I have a few questions regarding question 4 regarding 4d, 4e, and 4k.
Just a quick background. A while back I was on a study permit and my application to extend the permit was refused as they saw me not being a genuine student since I took time off from my studies to work on my mental health. I worked with the school and they were helping me get back on track and I payed my tuition deposit for the following semester. The school contacts and I did not know at the time that I could attend school while my extension permit application was being processed. So since I was not attending school and didn't notify them, I'm assuming is why they didn't see me as a genuine student and refused my extension. They notified me that I didn't have status anymore and as such was required to leave Canada. They gave me a Voluntary Departing Form which I needed to give at the last port of exit. They mentioned as long as I voluntarily depart, no further action will be taken against me. I complied with instructions and left Canada. I am now back years later as a 6 month visitor.
So my questions are as follows
4d) Been refused refugee status, or an immigrant or permanent Resident visa (including a Certificat de selection du Quebec (CSQ) or application to the Provincial Nominee Program) or visitor or temporary resident visa, to Canada or any other country?
Question 2) Since I was refused a study permit extension, would I fall under this and have to tick the yes box?
4e) been refused admission to, or ordered to leave Canada or any other country?
Question 3) Would my extension refusal and them stating I'm required to leave Canada because I no longer have status as well as having me fill a Voluntary Departing form make me fall under ordered to leave Canada? Should I tick yes?
4k) had any serious disease or physical or mental disorder?
Question 4) I was diagnosed recently with a fatty liver disease. It's still early and thankfully reversible. The doctor told me it was not at a dangerous stage but it's still serious and required a full lifestyle change. I also was suffering from anxiety and depression, but was not diagnosed officially since I never went to get myself checked for either so not sure if this would even classify since I was never diagnosed by a professional. Would any of these have me ticking yes and which ones?
Question 5) There is a box to provide details about all the "YES" answers. How much information should I provide? Like should I be very detailed or keep it simple and straight to the point? They would already have the details of my permit extension refusal on file for example, so would I just simply say that the permit was refused or go into deep detail about it? Lastly, should I even mention my anxiety and depression even though I was not diagnosed with them by a professional?
