What are the requirements for Health Card??????

Hello Everyone,

I just came to Canada , I want to make my Health Card , Can anyone let me know what is the procedure and requirements for making Health Card ?
I just have another copy of COPR (with stamp on it when i landed here) that i received with my Passport/Visa.

You need to wait 3 months before you have coverage anyways, so there is no hurry to apply. You may as well go after 2+ months and not rush it.

You will need to show a "proof of residency" document. So you must have something from "list 2" of this page: http://www.forms.ssb.gov.on.ca/mbs/ssb/forms/ssbforms.nsf/GetFileAttach/014-9998E-82~1/$File/9998-82E.pdf . Your proof must be exactly as the page describes it and not any different. Don't go to apply until you have one of these, or you will be turned away.
