Hello,

1. if parents/grandparents on a super visa re-enter Canada with expired insurance, does the insurance needs to be renewed, or is the insurance just required for the initial super visa application?
2. if parents/grandparents on a super visa application for a visitor record to stay longer than two years, does the insurance needs to be renewed?

1. CBSA will sometimes verify that insurance is valid at the port of entry. Not having valid insurance can create problems at the border.
2. Your choice. Generally speaking, you would want to make sure they have insurance unless you're good being stuck with a bill of several $100K if something goes really wrong.

Can parents/grandparents enter Canada under visitor and apply for visitor record to switch to super visa inside Canada since the super visa is an outside application?

If on a supervisa one of the requirements is to have supervisa insurance. It’s not optional. If caught without it they may have trouble visiting in the future.
