If one renews the ohip card does the following apply to him/her?

"you are in Ontario for at least 153 days of the first 183 days immediately following the date you establish residence"

Not if you are already covered after establishment of initial residency. http://www.health.gov.on.ca/en/public/programs/ohip/

Eligibility
Ontario residents are eligible for provincially funded health coverage (OHIP). Generally, to be eligible for Ontario health coverage you must :

be a Canadian citizen, permanent resident or among one of the newcomer to Canada groups who are eligible for OHIP as set out in Ontario’s Health Insurance Act ; and
be physically present in Ontario for 153 days in any 12-month period; and
be physically present in Ontario for at least 153 days of the first 183 days immediately after establishing residency in the province; and
make your primary place of residence in Ontario.
You must however "be physically present in Ontario for 153 days in any 12-month period;".

What if one used the full time student outside of Ontario exception. Does that change anything?

This is what the website says..

I am frequently away from Ontario, due to my job/studies; am I eligible for OHIP coverage?
If your job or studies require you to leave Ontario frequently and you are unable to be present for 153 days in any 12-month period, you may still be eligible for OHIP coverage as a mobile worker or mobile student. To maintain your OHIP coverage, you should:

be able to provide acceptable documents that show that your work requires frequent travel in and out of Ontario or that your full-time academic program in Ontario requires travel outside of Ontario; and
be able to provide acceptable documents to show how you make your primary place of residence in Ontario (refer to Ontario Health Coverage Document List).
New or returning residents to Ontario who qualify as a mobile worker or mobile student are exempt from having to meet the 153-day in the first 183-day physical presence requirement immediately after establishing residency in Ontario.
If this applies to you, you can try to make a case. However they are very aware of fraud, so you had better be able to prove that it does... Note that it states "that your full-time academic program in Ontario".

I had a few friends who went through this. The rule is pretty straight forward. If you're completing full time studies outside of Ontario - you're out of luck and don't qualify for an exemption under this clause. If you are studying in Ontario and your program requires you for some reason to spend extensive time outside of Canada (e.g. research program required to complete your PhD means traveling to a foreign country a staying there for a while to complete your thesis) then you have a case for keeping OHIP.

They have this:

Absences Outside Canada

You may be eligible for continuous OHIP coverage during a longer absence when you are away for one of the following reasons :

Study outside of Canada
Work outside of Canada
Charitable work outside of Canada

To be eligible for continuous OHIP coverage during one of these absences, you must first have been physically present in Ontario for at least 153 days in each of the 12-month periods for 2 consecutive years immediately before the absence. Different physical presence requirements may apply if you are an Ontario student studying full-time outside of Ontario but within Canada, and you choose to continue your studies at an educational institution outside of Canada.

To confirm continuous OHIP coverage during any longer absence from Ontario, you should contact your local ServiceOntario centre before you leave the province. You will need to show a document explaining the reason for your absence as required; for example, a letter from your school, employer or sponsoring charitable organization.
You can find it at http://www.health.gov.on.ca/en/public/publications/ohip/travel.aspx

You should however have talked to OHIP before you left for your studies but since you didn't, there's no solution but suck it up and talk to them now and hope they don't get mad.
