wrong decision on spousal sponsorship

i applied permanent residency for my wife under in canada spouse sponsorship class last year , she got her medical done in feb this year , in June we got an email from visa officer that we haven't received your medical , please provide information if you have done your medical already , the deadline was one month to provide the medical proof , i and my wife we both submit the medical proof on the IRCC web form and after submitting it we got the confirmation emails also saying that we received your documents and we will send it to the responsible officer,

recently couple days ago my wife got refusal because we were not able to submit the medical proof within the deadline and visa officer didn't find any communication from us, which is weird because we both submitted the medical proof on the very next day when we got email to submit it, we are very disappointed !!

Raise a case specific inquiry and bring this to the visa office's attention and ask for the applciation to be reopened. Also, your wife should contact her MP to request help.

i already put an inquiry on IRCC web forum and explained everything , don't know if they will reopen the file

No. Appeals take a very long time. I would not file an appeal immediately. Again, I would raise a case specific inquiry and ask them to reopen your application based on the fact that the medical was in fact submitted - and have your wife contact her MP.

i submitted inquiry on IRCC web forum yesterday evening. she got refusal yesterday afternoon.

As scylla already said, contact your MP. That is your best option to get the app reopened quickly.
