When it comes to dates, be as accurate as possible. Look into old contracts or job offers, letters, etc. Try
to figure out the dates. If you have to guess, attach a cover letter explaining why you can't provide
the exact dates.
You can put the end date as the end date of your work authorization.
When you fill up the form and point the mouse on the end date section and pop window will actually tell you to write the end date as the end date of your work authorization.
I left the end date blank, which the form does let you do for the top row, and I've had no problems with AOR or eligibility, but admittedly, putting your (future) work permit end date sounds at least as good an option.
