Work permit issue date?

Hello,





I am applying for Public Policy work permit. While filling imm5710e. It asks issue date and expiry date of my work permit. ((( I received a replacement work permit due to damage on my first work permit which has new issue date than the original one that I received after my study ))). So , what issue date should i fill? The new latest one or the one I received after my study???. Thanks for any input

The Date of Issue can be found at the back of your NRIC. For National Servicemen, please approach your Unit Chief Clerk for the Date of Issue stated at the back of your NRIC. For new cards with a QR code, you can use the SGWorkPass app on your mobile phone to scan the QR code for the card's Date of Issue.
