Hi Friends

i am filling forms for work experience for my spouse
Spouse worked for 1 years in one hospital (2013 - 2014)
1 years in Second hospital (2014 - 2015)
1 year in third hospital (2015 - 2016)
from 2013 to 2016

Do i need to Mention all time period separately in background/Declaration form for Sponsoring spouse?

or i can write

2013 --- 2016 At Job

It asks for the name of the company. Therefore you must enter each period of employment with each different company/institution separately.

She also worked as Oncall in other hospitals too , so this ON-Call (part time) overlapping in some years, what to do with this?

Like Full time from 2014 to 2015 in one hospital
On-call also in another hospital from 2014 to 2015

I would also list that separately. Just attach another sheet with her name, the form name, the question she's answering, and her UCI if she has one on any extra pages.

Also, you can put ON CALL in that row for that job next to "Nurse/Health Aid/or whatever her position was there". It makes it clear why those jobs overlap.
