WHICH VERSION OF IMM5669 TO BE USED FOR PARENTS IMMIGRATION - 2016 APPLICATION

Hello all,
I have a question regarding parents sponsorship. I applied in 2016 and now I have been asked to submit medical, police certificate, rprf and signed imm-5669. Now the delima I'm facing is which version of IMM-5669 should I submit. If we go to parents sponsorship link on cic website the form that exists is IMM5669 (01-2017) E. This is the form which has to be validated digitally and digital signature is required. On the other hand if you to to link for all CIC forms there is another IMM5669 form which is from 12-2012 and is manual with manual signatures. Can anyone please help me which one should I use?

on the "find forms and applications" cic's link both forms are available: One says

IMM 5669 - schedule A - Background/Declaration - To be used only with kits IMM5289 and IMM5772 - December 2016 (This is the latest one)

Other says:

IMM 5669 - schedule A - Background/Declaration - Deecmber 2012

Please help
