WHICH FORM TO FILL UP TO SEND MP

hi is any body know which form my wife has to fill up send it to MP???

and do i need to Sign that form to..??

I am not aware of any required form to send to your MP. Go to your MP's website as sometimes they have a form in a certain format they prefer. Otherwise just call them or send a letter or email explaining your issue and telling them your request.

ya my wife sent an e-mail and MP office told her that they will contact her back in 6 week ..

i ask about form b'cos i read somewhere that you might have to file the form for "Release of Information to an Individual".. so i had this query ...

I understand better now. That form is IMM 5475. I'm not sure you always need it but just in case.

Can we really get some help from MPs?

MPs have special numbers they can call to inquire about your file. My previous MP told me of our news months and months before we actually received it. MPs can find out of documents were received and help you to communicate if needed. Actually, it's not so much the MOs exactly but their assistants at the constituency offices. Help from MPs is limited. They can't sway the outcomes or decisions, just so you know.
