For sponsoring a spouse, outland application. On the sponsor questionaire - IMM 5540 - number 4 - provide the following details of your father and mother.
My mothers's maiden name is shown on my birth certificate and my marriage certificate, but her last name is legally her married name.
1. Which name should I use - her married name, or her maiden name.

2. If I use her married name, then should I put a note explaining that her maiden name is used on the above mentioned places?

If your mother's maiden name is the one that appears on all official papers then you should probably use that in the application, adding a note saying that she has another name as married.

I would recommend putting her legal name instead, and explaining that discrepancy... But I can't tell you for sure which option is right or wrong.

Well now I have been advised both ways, I guess maybe either way will be fine, just adding a note of explanation. Any other thoughts?
 
I would recommend to use that name that appears on her passport and id card. you can always put another name in the brackets next to another name.

Your mother's surname will be the same as your father's and her given name would be her maiden name. It would read as:

lastname firstname maidensurname
 
Thanks so much job-seeker. Now that you explained it that way it makes total sense. I think I had tunnel vision on that one.
