When is IMM 5604 needed? HELP PLEASEEEE!!!!

PA ( your son) will need to fill out form imm 5604. The requirement for age over 18 to fill imm5604 is for dependents. If PA is a minor, form imm5604 is still needed.

Should have information of father and mother (regardless of immigration status) . (and siblings if any)
